What is a health risk score?

A health risk score is a single number that estimates how likely a patient is to experience a specific negative outcome—such as a complication, hospital readmission, or chronic disease progression—based on their age, medical history, test results, and lifestyle factors. Think of it as a predictive tool, not a diagnosis: the system weighs various pieces of information and outputs a score that flags the patient as low, moderate, or high risk. Hospitals use these scores to decide who needs closer monitoring, extra resources, or preventive interventions, and to help plan staffing, bed capacity, and preventive care programs. Keep in mind that different scoring systems exist for different purposes, and their accuracy depends heavily on the quality and completeness of the patient data they’re built on.
